COMMUNITY HABILITATION (Com Hab)

Community Habilitation promotes independence and community integration by offering skills training and support to individuals who reside at home. It includes the acquisition, retention, and improvement in skills related to life, safety, activities of daily living, and social skills, allowing individuals to live as independently as possible in their natural home environments.

REQUIREMENTS



Must have a developmental disability
Must have Medicaid and reside in Long Island.

IN-HOME RESPITE
This program provides a temporary respite relief for the parent and/or caregiver, which helps reduce overall family stress.

RECREATION RESPITE (After School Program)
This program offers individuals a structured social and recreational environment. Individuals have an opportunity to increase their socialization and interpersonal skills through sports, games, arts and crafts, music, and other recreational activities.

This program is located in the Westbury School District, Park Avenue School. Hours of operation are Tuesday, Wednesday, and Friday from 3:00 PM - 6:00 PM. Transportation to the site may be provided by the school district. Parents or caregivers are responsible for picking up their individuals promptly at 6:00 PM.

WEEKEND RESPITE (Recreation Program)
This program allows individuals to participate in a wide range of activities, while experiencing different recreational and social environments. Thus providing a much needed respite break for the primary caregiver. Hours of operation are from 10:00 AM to 3:00 PM. The program is located at St. John?s Episcopal Church, 536 South Franklin Street, Hempstead, NY 11550.
